Microscope-assisted treatment

An operating microscope reveals micro‑details of canal anatomy, enabling conservative shaping, enhanced disinfection, and reliable obturation. We follow standardized protocols and document key steps.

What it is

  • 6–25× magnification with coaxial light — visualize canals, cracks, and perforations.
  • Conservative access & shaping — tissue preservation, controlled taper and working length.
  • Chemomechanical cleaning — irrigant activation (ultrasonic/sonic/negative pressure).
  • Hermetic obturation — warm gutta‑percha or bioceramics/MTA when indicated.
  • Strict asepsis — rubber dam isolation and sterile barriers as standard.
  • Documentation — photo/video and stepwise radiographs as needed.

Why it’s needed

  • Locating extra canals (e.g., MB2) and variant anatomy.
  • Retreatment of insufficiently filled/infected canals.
  • Removal of posts, cement, or separated instruments.
  • Perforation repair and apical management with MTA/bioceramics.
  • Pre‑prosthetic optimization for teeth planned for crowns/bridges.
  • Reducing risks of complications and future re‑interventions.

How it goes

  1. Assessment
Exam, RVG/CBCT if indicated; percussion/palpation; vitality tests; define the plan.
  2. Isolation
Rubber dam and antiseptic field control.
  3. Access & navigation
Microscope‑guided access; identify and verify all canals.
  4. Shaping
Endomotor NiTi with curvature control; taper/length management.
  5. Irrigation & activation
NaOCl/EDTA, ultrasonic/sonic activation or negative pressure.
  6. Reconstruction
Temporary seal or core build‑up; MTA repair when needed.
  7. Obturation
Warm vertical compaction (down‑pack/back‑fill) or bioceramic techniques.
  8. Control
Radiographic check, instructions, scheduling definitive restoration.

Indications

  • Failed previous RCT (pain, sinus tract, periapical lesion).
  • Complex anatomy: curvature, calcifications, extra canals.
  • Separated instrument, perforation, or the need to remove a post.
  • Teeth planned for high‑load prosthetic restorations.
  • Suspected vertical root fracture (with appropriate diagnostics).

Aftercare

  • Mild soreness up to 2–3 days — take NSAIDs/analgesics if needed.
  • Chew carefully until the definitive restoration; avoid hard/sticky foods.
  • Keep follow‑ups and complete the final crown/onlay when indicated.
  • If pain/swelling worsens or a sinus tract appears — contact us promptly.

FAQ

Is a microscope always necessary?
Not always. For complex anatomy, retreatment, post removal and perforation control it is the gold standard.
How long does it take?
Typically 60–120 minutes; complex retreatments may require two visits.
Is it painful?
Performed under reliable anesthesia (STA/nerve block); discomfort is minimal.
Risks and limitations?
Root cracks, severe calcifications, or extreme curvatures may limit prognosis; we discuss these before treatment.
Will I need a crown afterwards?
Posterior teeth with large loss of structure benefit from cuspal coverage (onlay/crown) to prevent fractures.
Retreatment prognosis?
Depends on the initial status; microscope significantly improves visualization and outcomes.
Same‑day definitive filling?
Possible in primary cases with sufficient tooth structure; otherwise temporary build‑up with later cuspal coverage.
Is it safe during pregnancy?
When indicated — yes; gentle protocols, X‑rays only if necessary, discussed individually.
How much does it cost?
Varies by tooth group and complexity; the page below shows official price list items unchanged.
